The Langenbeck is the everyday hand-held retractor. Its right-angled blade hooks over a wound edge and the assistant applies tension through the handle, giving direct control over how hard and in what direction the tissue is held. Because incisions vary so much, it is supplied in a graduated range of blade widths and depths rather than one size, and most sets carry several.

The blade tip is turned and polished so it presents a smooth, radiused edge to tissue. A sharp or poorly finished blade edge concentrates pressure and bruises the wound margin, which is the most common complaint about cheaply made retractors.
