A punch biopsy is the workhorse diagnostic procedure of dermatology. The circular blade is placed on the lesion and rotated under downward pressure until it cuts through dermis into subcutaneous fat, producing a cylindrical core that preserves the full architecture of the skin — which is what the pathologist needs to distinguish inflammatory patterns and tumour depth.
Diameter is chosen by purpose: 2 mm for cosmetically sensitive sites, 3–4 mm as the diagnostic standard giving enough tissue for reliable interpretation, and 5–8 mm for excisional use. Blade sharpness is critical because a dull punch crushes the specimen and produces artefact the pathologist cannot read past.
