The Lahey grips tissue that has to be pulled rather than merely held. Its sharp curved tips penetrate and lock into firm tissue such as the thyroid, and the ratchet holds the traction so the surgeon can work with the gland drawn into view. The same pattern is used in general surgery wherever firm, sustained traction on a solid structure is needed.
