Myringotomy drains middle-ear effusion and allows ventilation tube placement. The incision must be precise: made in the correct quadrant of a structure only a few millimetres across, deep enough to divide the membrane fully but not so deep as to damage the ossicles or promontory beyond it. That combination of a very fine point and absolute control of depth is what the instrument is built for.
Blade sharpness matters more here than almost anywhere else — a dull blade pushes the membrane inward rather than incising it, which both fails to drain and risks injuring middle-ear structures. Many patterns use disposable blades for this reason.
