Narrowing the bony nasal vault requires cutting the nasal bones along defined lines so they can be infractured into a new position. The Converse osteotome is scaled for that: narrow blades, often with a guard to protect the overlying skin and mucosa, driven with a light mallet along a planned osteotomy line. Guarded and unguarded patterns are both used depending on approach.
Blade width and the guard determine how precisely the cut follows the intended line. Because nasal bone is thin, an osteotome that deflects produces a comminuted rather than clean fracture, which compromises the aesthetic result.

Mirror and satin finishes are both available — satin is usually specified where glare under operating lights is a concern.
