Cusco Specula — Pattern and Clinical Use
The Cusco is the everyday speculum. Two hinged blades are inserted closed, then opened by a lever and locked at the required separation with a thumbscrew, so the clinician has both hands free for a smear, swab, colposcopy or IUD insertion. The self-retaining lock is what distinguishes it from a Sims pattern and makes single-operator examination practical.
The lock must hold reliably under the natural closing pressure of the vaginal walls. A speculum that creeps closed during a procedure loses the view at the worst moment, so thumbscrew thread quality is a functional rather than cosmetic characteristic.
