The cervix must be held steady and straightened before instruments are passed into the uterus. The tenaculum’s sharp points penetrate the cervical tissue and the ratchet holds the grip, letting the operator apply traction that aligns the cervical canal with the uterine cavity — which is what reduces the risk of perforation during sounding, dilation or IUD insertion.

Point sharpness matters because a blunt tenaculum tears rather than penetrating, causing more bleeding and a less secure hold. Single-tooth patterns concentrate the grip; multi-tooth patterns spread it and cause less trauma.
