The weight is the mechanism. With the patient in lithotomy, the blade is placed against the posterior vaginal wall and the weighted handle hangs down, holding retraction by gravity alone with no assistant and no locking mechanism. That frees the whole surgical team during vaginal hysterectomy, repair procedures and D&C, which is why it remains standard despite its simplicity.
Weight mass and balance determine whether retraction holds — too light and the blade rides up, too heavy and it drags uncomfortably. Many patterns include a drainage channel or attached bag to clear fluid from the field.

Mirror and satin finishes are both available — satin is usually specified where glare under operating lights is a concern.
