A fibroid is firm, smooth and difficult to grasp with any conventional forceps — it simply slips. The myoma screw is rotated into the substance of the fibroid so the helical thread engages the tissue, giving a secure hold for the traction needed to enucleate it from its bed. It converts an ungraspable mass into something the surgeon can pull on.
Thread pitch and sharpness determine whether the screw engages or simply bores a hole. The shaft must also take substantial torsional and tensile load without bending, because considerable traction is applied through a narrow instrument.
